• Has your organization ever done business with us? If so, when, and what?
• Has the individual ever done business with us? If so, when, and what?
• How will the requested donation be used to benefit your organization/individuals?
• Have you received a donation from us in the past?
• Attach any supporting documentation: letters, forms, etc.
